ASQ-3 will be due for each child. New enrollees will be required to complete within the first 90 days of care.

The following are the links to your child's ASQ-3 Questionnaire. Print & read through it thoroughly before completing with your child. If your child has not demonstrated any of the tasks, try it with them and see how they score. After completion, give us the printed copy & we will score it and set up a brief conference to discuss scoring. If you have any concerns, feel very free to call us for help. Enjoy!!!
